Putting Joined-Up Care Into Practice

Many of our clients have complex or ongoing health needs. Whether they’re recovering from a hospital stay, managing a long-term condition, or receiving palliative care, they often require both clinical and personal care at home.

Here’s how we work hand-in-hand with healthcare professionals:

 

District Nurses

District nurses play a key role in delivering medical care at home—wound care, injections, catheter support, and more. We ensure clear communication with the nursing team, so our carers can follow any medical guidance and support clients in between visits.

For example, if a district nurse is monitoring a pressure sore, our carers will ensure correct positioning, keep the area clean, and report any changes.

 

GPs and Clinical Teams

We keep in contact with GP surgeries and community health teams when needed. With consent, we can help relay important updates and support clients in managing appointments, medication regimes, and follow-up instructions.

 

Hospital Discharge Teams

We often support individuals coming home after hospital stays. We liaise with discharge planners and therapists to ensure the care package we deliver is ready from day one, reducing the risk of readmission.

 

Occupational Therapists & Physiotherapists

We help implement guidance from OTs and physios—whether it’s assistance with mobility exercises, supporting safe transfers, or using equipment correctly at home.
