NHS Continuing Healthcare (CHC)
For individuals with more complex, ongoing health conditions, NHS Continuing Healthcare may be available. This is a fully funded package of care, provided by the NHS.
To be eligible, you’ll need to go through a clinical assessment to determine whether your needs are primarily health-based.
If you’re approved, CM Bespoke Care can provide care through your CHC package, ensuring consistency and compassionate support at home.
